How we work

The HHS Office for Civil Rights Breach Portal is the federal regulatory record of file. We treat it as authoritative on its own for the facts it contains: organization name, affected count, breach type, and submission date. Additional non-government claims (data elements exposed, attacker attribution, ransom demands, dark-web leak status, class-action filings) require at least one corroborating source before they appear on a page: the breached entity's own notification, a state attorney general filing, established trade press (HIPAA Journal, Becker's, DataBreaches.net), or a class-action plaintiff filing. We do not write from memory. We do not invent details. When a source is missing for a non-government claim, we say so explicitly on the page.
